What is HPC?

HPC strives to empower clients to attain financial independence and preserve their housing.  It provides personalized case management to assess each individual situation and connects clients with the resources required to help with the challenges they may be facing.

The Community Support Liaison will help clients:

  • Identify and address issues that affect health, housing and self sufficiency,

  • Connect with other programs, agencies and nonprofits to provide the most effective and beneficial resources,

  • Apply for fuel assistance, housing programs and funding, as well as other benefits,

  • Develop and manage a budget,

  • Connect with mental health and substance use services, if needed,

  • Identify, set, and achieve goals to become more self-sufficient and thrive.
